Referrals

• Referring Physicians may complete the form below for any patients that require the consult of our otolaryngology physicians.  
• Patients with any form of Medicare or Medicaid REQUIRE a referral from their primary physician.  
• Even if the patient has private insurance that doesn't state a referral is required, we still request that you have your PCP or Urgent Care send us a referral for history of diagnosis & prior treatments.
• Please allow a minimum of 3 Days for faxed referrals sent to our referral coordinator to allow time to contact the patient with appointment date & time.  
• We will send appointment information back to referring office once setup.
