Referrals

• Referring Physicians may complete the form below for any patients that require the consult of our otolaryngology physicians.  
• Patient's with any form of Medicare or Medicaid require a referral from their primary physician.  
• If patient has private insurance that doesn't state that a referral is required you may contact our office at (478) 272-8382 to set up an appointment over the phone.
• Please allow 48hrs for faxed referrals sent to our referral coordinator to allow time to contact the patient with appointment date & time.  
• We will send appointment information back to referring office once setup.
